‘Worst case of abuse I have ever seen’: Inspector says state’s report of nursing home patient’s death was falsified

Wilmington, DE - The state’s investigation started in late January 2004. A registered nurse who inspected nursing homes for the state for four years spent several weeks at Shipley Manor, checking out the complaint. She surveyed the facility, interviewed staff, checked records and talked to a sample of its residents.
Dianne Roberts, who did the survey, said she considered Teed Autman’s condition “the worst case of abuse I have ever seen.”But according to Roberts, federal officials got a different report from the state. And, she said, the changes made in that report were part of a pattern of interference she saw while working as a state surveyor.
