Earlier report cited flaws at vets’ home

Phoenix, AZ - A federal inspection of the state’s nursing home for veterans turned up shortcomings in care and other areas, including some problems already publicly reported from a separate survey conducted soon afterward.

A 42-page checklist produced by a 13-member team from the Department of Veterans Affairs from a routine Jan. 22-29 inspection found problems with inattention to call buttons, failures to update assessments of patients’ needs, missing documentation for treatment and numerous instances where residents’ individual nutritional needs weren’t met.
