Surveys show deficiencies grew

Acton, MA - When Julia McCauley tumbled to her death down the foyer staircase at Life Care Center of Acton one morning in 2004, it marked the beginning of a growing list of concerns from state officials and assisted living facility advocates about the wellbeing the nursing home’s residents.

In the next three years, the home would receive more critical marks on state surveys than most of its sister facilities in the Bay State and face fines for non-compliance of state guidelines for elder care, in some cases leading to residents being harmed, according to state records.
