Woman, 100, dies after fall: Nursing-home accident under investigation

Lancaster, OH - Fairfield County authorities are investigating the death of a 100-year-old nursing-home resident who fell Sunday in front of the facility.
Edith West was sitting in a wheelchair in front of Heartland Fairfield Nursing Center near Pleasantville when she rolled into the parking lot and fell out of the chair, Sheriff Dave Phalen said.
The nursing home reported the fall Monday to the Ohio Department of Health, a requirement after any incident involving suspected neglect or abuse.
