Violations threaten nursing home’s funds

Phoenix, AZ – An Ahwatukee nursing home has until March 28 to fix problems that pose a "serious threat" to patients or its Medicare and Medicaid funds will be pulled.

And if it doesn’t remedy state law violations within six months, it could be closed down completely – a warning the state hasn’t issued to a nursing home in five years.

Evergreen Foothills Health and Rehabilitation Center was found to be in violation of 23 state laws during a December Arizona Department of Health Services investigation, state documents show. Violations were also found during investigations in 2005 and February 2007.
