State suspends new admissions to Palmyra nursing home: Palmyra Health Care Center cited for multiple violations

Clarksville, TN – Palmyra Health Care Center nursing home is forbidden from accepting new residents until it corrects multiple violations, as sanctioned by Tennessee Department of Health Commissioner Susan R. Cooper.

The suspension has been in effect since Monday, according to a media release. The nursing home, located at 2727 Palmyra Road, will have to pay a state civil penalty of $3,000 in addition to a federal civil penalty of $5,250 per day, the release said.

The violations were discovered during a complaint investigation and annual survey conducted March 31 through April 2, the release said.
